Red Sox 3, Yankees 2: Rafael Devers saves the day in the ninth

In a freshly tied game in the ninth, Reed issued his own leadoff walk. He recovered, though, even after his manager attempted to pull him in the middle of an at-bat. Farrell came out of the dugout to insert Craig Kimbrel once Reed fell behind Jacoby Ellsbury, but he was prohibited from doing so because the team had already made a mound visit during the at-bat.

“I messed up. I know the rule,” Farrell said. “I messed up trying to get Kimbrel in the game.”

So my question is does the pitcher have to finish the at bat once a coach has gone to the mound or is the manager simply prohibited from making a visit? Is there any reason Farrell couldn't have just said "fine I won't visit, Addison come on in Kimbrel is pitching now." I assume not but I've never seen that anywhere before.

What a pathetic display of managing. For a while there it looked like Farrell had given Kimbrel the weekend off.

EDIT: Heck, DD should have pulled a Ray and shitcanned Farrell in between innings.

Yeah but is there any reason he just can't pull Reed from the dugout? Why does he need to visit the mound to do that?

I didn't see anything immediately in the rule book stating that the pitcher can only be substituted for through a coaching visit. There is a comment in the above rule:

In a case where a manager has made his first trip to the mound and then returns the second time to the mound in the same inning with the same pitcher in the game and the same batter at bat, after being warned by the umpire that he cannot return to the mound, the manager shall be removed from the game and the pitcher required to pitch to the batter until he is retired or gets on base. After the batter is retired, or becomes a base runner, then this pitcher must be removed from the game. The manager should be notified that his pitcher will be removed from the game after he pitches to one hitter, so he can have a substitute pitcher warmed up.
I infer from this that it's the intent of the rules to have a pitcher complete his turn against a batter if he's already been visited during that at-bat.

I guess if a pitcher gets injured, then the pitcher has to issue an intentional walk (somehow)? Interesting that automatic intentional walk actually would address the situation where a manager makes a trip to the mound, then in the same at bat the pitcher becomes irrevocably injured (say he charges into foul territory to stop a bunt from becoming fair and collides with the catcher and gets hurt).

And I'm sure the intent of the rule is to prevent any extra time wasting for the reliever who wasn't ready at the start of an at-bat.
